Application Process
Once we receive your application you will receive a confirmation email. After we confirm we have received your application we will not respond to any update requests as this takes time away from reviewing the applications.
Once your application is reviewed you will be mailed a letter informing you of the status of your application.
If you are approved and we have an available volunteer dentist, before we provide you with the dentist’s contact information, you will receive a form confirming your contact information. This form needs to be returned within two weeks. If you return this form to us, you will receive your dentist’s information via email within 2 weeks of receipt of the form.
Once you receive your dentist’s information you must call within 30 days of receipt to schedule your appointment.
The consultation appointment with the dentist is the final step in the eligibility screening process. At your consultation with the dentist they will determine whether they can perform the dental work that you need and from a clinical perspective, confirm that your needs fit the program.
If your needs are within the program and dentist’s scope, the dentist will also work with you to develop a treatment plan based on what they are able to donate at that time. If you do not accept the treatment plan offered by the dentist, you will not be matched with a new dentist and your case will be closed. If your dental situation is beyond the scope of the program, your case will be closed and you will not be matched with a new dentist.
If your dental situation is beyond the scope of the program, your case will be closed and you will not be matched with a new dentist.
